Drug-related harms in patients with polypharmacy are often not identified as such by physicians, which can lead to further harm from cascade prescribing. People on multiple medicines require holistic and generalist overview, with a medicines-focused approach, as well as specialist management. This talk looks at the harms and challenges associated with problematic polypharmacy, and provides practical tips on how to identify and address them. We will cover the factors that should be considered when deciding whether or not to continue a prescription, including assessment of likely benefits and harms, and emphasis on a personalised approach where evidence base is lacking.
