Acute back pain is an uncommon problem on the medical take, but when they present, lead to clinical challenges. It is important to recognise red flag symptoms and signs and the role of urgent MR imaging. Vertebral fragility fractures are the  most common cause for admission in older people but their care in suboptimal.  Sacral fractures are another group of patients, but these are rarely diagnosed.
