We have an ageing population with people living longer with chronic diseases and multiple comorbidities. Identifying that someone is approaching their “last year of life” is challenging. People with long-term conditions can be admitted repeatedly to hospital, although this may not be what they would want if asked. In acute settings, active treatment is often the default course of action.
