Tackling the perils of polypharmacy

Streams: Clinical Pharmacology
In the UK, over 1 billion prescription items are dispensed in the community each year and >1 million people are on 8 or more medicines. Use of multiple medicines (polypharmacy) is associated with increased risk of adverse reactions, hospital admission, medication errors and lack of adherence to treatment. This presentation will consider the natural history of polypharmacy and provide pragmatic guidance on primary prevention. Medicines optimisation in patients with established polypharmacy will be illustrated using case studies.
Learning outcomes:
Delegates who attend this session will be able to:
  1. Define polypharmacy and describe its clinical consequences
  2. Describe risk factors for polypharmacy
  3. Use pragmatic approaches to prevent polypharmacy
  4. Identify common drugs that can be stopped in different clinical scenarios and understand the benefits and risks of these actions
